This form is for enrolment in the "short courses" listed on the next page. These courses are generally single units of competency or skill sets from a nationally accredited training package.

All students must provide as part of completing this form, and bring to the course if it is classroom based; one of the following forms of Photo Identification:
Driver or Rider Licence or permit issued by an Australian
State or Territory or any foreign country
Australian or other passport
NSW photo card
Proof of age card issued by a public authority of the
Commonwealth or of another State or Territory for the
purpose of attesting to a person's identity and age
Keypass (over-18) identity card issued by Australia Post.

This form may be used for enrolment in the Targeted Priorities Prevocational & Part Qualifications program. These programs are fully subsidised by the NSW Government and eligibility criteria under Smart and Skilled apply.

For example, the Our Shout for an RSA Initiative has the following eligibility requirements:

Eligible students must:

meet the Smart and Skilled NSW residency and citizenship criteria and no longer be at school as per the eligibility policy,

be aged 17 or over, and

must not have previously held an RSA competency card that has expired and is currently due for renewal.

Upon receipt of this fully completed form and other pre enrolment requirements eg. LLN Quiz, USI Transcript access; Austrain Academy will check availability and your eligibility and contact you to advise if your enrolment application was successful.

Please Note - for RSA Enrolments - Victorian, ACT or Northern Territory residents who hold a current RSA in their home state and are seeking to temporarily work in NSW, can now do so under the new automatic mutual recognition agreement. If they move their principal place of residence or work to NSW, they will need to apply under the existing mutual recognition pathway. Applicants from SA, TAS, QLD or WA seeking to work in NSW either temporarily or permanently, must apply under the existing mutual recognition agreement. These states have yet to adopt the automatic mutual recognition agreement. Additionally, if you wish to become a licensee, approved manager or club secretary in NSW you may only need to complete the Licensee training.

The units of competency are nationally accredited but the units are customised to also meet a NSW licensing/compliance outcome.

COVID-19 Information for students: Austrain Academy will continue to monitor the situation closely and take advice from NSW Health to ensure we are responding to the situation appropriately. The following requirements will be in place for all face to face classroom training:

Recognition of Prior Learning (RPL) & Referral Pathway

Recognition of Prior Learning or RPL is an assessment process that assesses the participant's skills, knowledge and experience gained through work, formal learning and informal learning to determine the extent to which that participant has achieved the required learning outcomes or competency outcomes.
Credit transfer is a process that provides participants with agreed and consistent credit outcomes for components of a qualification based on identified equivalence in content and learning outcomes between matched qualifications. Credit transfer is applicable for the equivalent Training Package Units only.
I hereby apply under the provisions of the VET Quality Framework for the knowledge and skills I have previously acquired to be assessed and the appropriate recognition to be granted to me.

Fee Free - Smart & Skilled Part Qualifications

Students undertaking a part qualification will not be charged a fee as these programs are fully subsidised by the NSW Government. Completing a part qualification under the Targeted Priorities Prevocational and Part Qualifications Program does not affect the fee eligibility of a student undertaking a full qualification under Smart and Skilled.

Where a student, as a result of undertaking two or more part qualifications under the Targeted Priorities Prevocational and Part Qualifications Program is awarded a full qualification, and subsequently undertakes a full qualification under the Entitlement or Targeted Priorities Full Qualifications program, and is eligible to pay a standard student fee, the student must pay the Standard Student – Subsequent Qualification Fee or the concession fee if a fee exemption is not applicable. The student must indicate at enrolment that they have previously achieved a full qualification.

Unique Student Identifier (USI) Information

From 1 January 2015, Austrain Academy can be prevented from issuing you with a nationally recognised VET qualification or statement of attainment when you complete your course if you do not have a Unique Student Identifier (USI). This applies whether you are doing a full Qualification or a single Unit of Competence. If you have not yet obtained a USI 
you can apply for it directly at https://www.usi.gov.au/your-usi/create-usi on computer or mobile device. Please note that if you would like to specify your gender as ‘other’ you will need to contact the USI Office for assistance.
If you have a USI, you must provide it to Austrain Academy for verification with the Student Identifiers Registrar. If you have lost your USI, you may authorise Austrain Academy to locate it.

We will use the number you provide as well as the name and date of birth and number from one of the forms of identification in Section 14. This identification information must 
be the same as when the USI was created. You may access your USI account and provide permission for Austrain Academy to view your training records/transcript to assist with an application for Recognition of Prior learning (RPL). You may also give Austrain Academy permission to view and update your account details.  See www.usi.gov.au/students for more information.

You are advised and agree that you understand and consent that the personal information you provide in connection with your application for a Unique Student Identifier (USI):
-is collected by the Registrar as authorised by the Student Identifiers Act 2014.

current and former Registered Training Organisations to enable them to deliver VET courses to the individual, meet their reporting obligations under the VET standards and government contracts and assist in determining eligibility for training subsidies;
schools for the purposes of delivering VET courses to the individual and reporting on these courses;
the National Centre for Vocational Education Research for the purpose of creating authenticated VET transcripts, resolving problems with USIs and for the collection, preparation and auditing of national VET statistics;
-researchers for education and training related research purposes;
any other person or agency that may be authorised or required by law to access the information;
any entity contractually engaged by the Student Identifiers Registrar to assist in the performance of his or her functions in the administration of the USI system; 
and will not otherwise be disclosed without your consent unless authorised or required by or under law.
The consequences for not providing the Registrar with some or all of your personal information are that the Registrar will not be able to issue you with a USI.

Why we collect your personal information

As a registered training organisation (RTO), we collect your personal information so we can process and manage your enrolment in a vocational education and training (VET) course with us.

Personal information is required for enrolment into our programs to carry out the functions and services of our organisation, and to satisfy eligibility and reporting requirements. If you do not wish to complete some forms or sections of required documentation, please advise an Austrain Academy representative and they will inform you if that information is required for enrolment into a specific program.

How we use your personal information

We use your personal information to enable us to deliver VET courses to you, and otherwise, as needed, to comply with our obligations as an RTO.

How we disclose your personal information

We are required by law (under the National Vocational Education and Training Regulator Act 2011 (Cth) (NVETR Act)) to disclose the personal information we collect about you to the National VET Data Collection kept by the National Centre for Vocational Education Research Ltd (NCVER). The NCVER is responsible for collecting, managing, analysing and communicating research and statistics about the Australian VET sector.

We are also authorised by law (under the NVETR Act) to disclose your personal information to the relevant state or territory training authority.

How the NCVER and other bodies handle your personal information

The NCVER will collect, hold, use and disclose your personal information in accordance with the law, including the Privacy Act 1988 (Cth) (Privacy Act) and the NVETR Act. Your personal information may be used and disclosed by NCVER for purposes that include populating authenticated VET transcripts; administration of VET; facilitation of statistics and research relating to education, including surveys and data linkage; and understanding the VET market.

The NCVER is authorised to disclose information to the Australian Government Department of Education, Skills and Employment (DESE), Commonwealth authorities, State and Territory authorities (other than registered training organisations) that deal with matters relating to VET and VET regulators for the purposes of those bodies, including to enable:

·         administration of VET, including program administration, regulation, monitoring and evaluation

·         facilitation of statistics and research relating to education, including surveys and data linkage

·         understanding how the VET market operates, for policy, workforce planning and consumer information.

The NCVER may also disclose personal information to persons engaged by NCVER to conduct research on NCVER’s behalf.

The NCVER does not intend to disclose your personal information to any overseas recipients.

DESE is authorised by law, including the Privacy Act and the NVETR Act, to collect, use and disclose your personal information to fulfil specified functions and activities.      For students enrolling in subsidised training under Smart and Skilled: Consent to use and disclosure of personal information

Training Regulator (Data Provision Requirements) Instrument 2020, Austrain Academy is required to collect personal information (information or an opinion about me), collected from me, my parent or guardian, such as my name, Unique Student Identifier, date of birth, contact details, training outcomes and performance, sensitive personal information (including my ethnicity or health information) and other enrolment and training activity-related information (together Personal Information) and disclose that Personal Information to the National Centre for Vocational Education Research Ltd (NCVER).

My Personal Information (including the personal information contained on my enrolment form and my training activity data) may be used or disclosed by Austrain Academy for statistical, regulatory and research purposes. Austrain Academy may disclose my personal information for these purposes to third parties, including:

School - if I am a secondary student undertaking VET, including a school-based apprenticeship or traineeship;

Employer - if I am enrolled in training paid by my employer;

Commonwealth and State or Territory government departments and authorised agencies, including the NSW Department of Education (Department);

NCVER;

Organisations (including the Department) conducting student surveys; and Researchers

Personal Information disclosed to NCVER may be used or disclosed for the following purposes:

issuing a VET Statement of Attainment or VET Qualification, and populating Authenticated VET Transcripts;

facilitating statistics and research relating to education, including surveys;

understanding how the VET market operates, for policy, workforce planning and consumer information; and

administering VET, including program administration, regulation, monitoring and evaluation.

I may receive an NCVER student survey which may be administered by an

NCVER employee, agent or third party contractor. I may opt out of the survey at the time of being contacted.

NCVER will collect, hold, use and disclose my Personal Information in accordance with the Privacy Act 1988(Cth), the VET Data Policy and all NCVER policies and protocols (including those published on NCVER's website at www.ncver.edu.au.

The department may disclose my Personal Information to other Australian Government agencies, including those located in States and Territories outside NSW.

The above government agencies may use my Personal Information for any purpose relating to the exercise of their government functions, including but not limited to the evaluation and assessment of my training, the determination of my eligibility to receive subsidised training or any Fee Exemptions or Concessions. My Personal Information may also be disclosed to other third parties if required by law.

I also acknowledge and agree that the Department may contact me by telephone, email or post, during or after I have ceased subsidised training with Austrain Academy for the purposes of evaluating and assessing my subsidised training.
